How to keep a bowel diary for your doctor
Keep a bowel diary for two to four weeks before the appointment. Each day, note time, Bristol stool type, urgency or pain if present, and anything obvious you ate or felt beforehand. Bring a one-page summary, not a stack of raw notes.

Keep a bowel diary for a clinic visit
- Pick a start date at least fourteen days before the appointment.
- After every bowel movement, record time and Bristol type (or Hard / Normal / Soft / Liquid).
- Add short tags for pain, urgency, bloating, or foods you want to test.
- Mark days with no bowel movement so frequency is honest.
- The day before the visit, write weekly averages and three questions you want answered.
- Export or photograph the summary so you can show it in the room.
What belongs in a bowel diary
Clinicians care about frequency, stool form, blood or mucus, night waking, and how symptoms relate to meals or stress. The IFFGD-style daily diary also leaves room for medicines and mood. You do not need a novel. You need consistent fields.
If you only manage three fields, choose time, Bristol type, and one symptom score. That alone beats vague recall.
Paper chart versus a poop tracker app
Paper templates from continence clinics still work. An app helps when you are away from home, when you want type distribution charts, or when you need a CSV for the notes. Doody’s New Entry screen is built for that bathroom-moment logging: type tiles, food tags, symptom tags, save.
Whatever tool you use, the rule is the same: log on calm days as well as flare days. Baseline is what makes a bad week visible.

Count bowel movements per day (range and average). Note the most common Bristol types. List how many days had urgency, pain, or liquid stool. Add two or three suspected triggers and what you have already tried.
Hand that half-page over first. Offer the full log if they want detail. You spend the visit on decisions instead of reconstructing last Tuesday.
FAQ
How many days should a bowel diary cover?
Fourteen days is a practical minimum. Twenty-eight days is better if your symptoms swing with your cycle or from week to week.
Should I log when nothing happens?
Yes. “None” or blank BM days are part of constipation and frequency assessment. Skipping them makes you look more regular than you are.
